1. Acceptance
By using this website, submitting the intake form, or signing up as a chiropractor partner, you agree to these terms. If you don't agree, please don't use the platform.
2. What the platform is — and isn't
This is an information-collection and lead-routing service. It connects patients with licensed chiropractors. It does not diagnose, treat, or provide medical advice, and nothing on this site should be treated as a substitute for professional medical evaluation. If you are experiencing a medical emergency, call 911.
3. For patients
- You confirm the information you submit is accurate to the best of your knowledge.
- Submitting the form does not create a doctor-patient relationship — that begins when the chiropractor partner contacts and evaluates you.
- We aim to route your submission promptly but do not guarantee a specific response time beyond the "within 2 business hours" contact target shown at submission.
4. For chiropractor partners
- Subscriptions are billed monthly in advance via the payment method on file, at the rate for your selected plan.
- Your plan's monthly price includes a set number of cases (or unlimited, on the Unlimited plan); cases beyond that are billed at the per-case overage rate shown on your plan at signup, reviewed and charged by our team each billing cycle.
- Territory (ZIP code) exclusivity applies only while your account is active and in good standing.
- You may cancel at any time from your billing dashboard; cancellation takes effect at the end of the current billing period.
- Use of patient information routed to you is governed by the Business Associate Agreement and applicable law — for care coordination and evaluation scheduling only.
- Accounts may be suspended for non-payment, misuse of patient information, or violation of these terms.
5. Prohibited use
- Submitting false or fraudulent intake information.
- Attempting to access another partner's leads or territory.
- Using the platform or routed patient information for any purpose other than care coordination.
- Circumventing security controls (rate limiting, bot detection, authentication).
6. Disclaimers & limitation of liability
The platform is provided "as is." We do not warrant that referrals will result in treatment, insurance coverage, or any particular outcome. To the fullest extent permitted by law, our liability for any claim relating to the platform is limited to the fees you paid in the three months preceding the claim.
7. Changes
We may update these terms; continued use after an update constitutes acceptance of the revised terms.
